Use this dedicated prescription when the case has been assigned to the YM Prestige team for esthetic planning and fabrication.
One prescription per physical case. Digital case files and protected patient information belong in the client portal or secure upload, not a general contact form.
Use YM unless the restoration was specifically planned and accepted as a Prestige case.
Include the prescribed restoration, tooth, material, shade, due date, and case-specific direction.
Pack the printed slip with a physical case, or use the established secure workflow for digital records.
