Scope
This policy applies to information submitted through the public pages and forms on ymdental.com. The public website is intended for business inquiries, practice onboarding, education, pickup coordination, and general laboratory information.
Do not submit patient names, prescriptions, scans, medical records, protected health information, payment-card information, or case files through a public website form. Use YM's approved case systems and secure clinical workflows for those materials.
Information we collect
Depending on the request, YM may collect business contact details such as a name, role, practice or organization, email address, operational phone number, office address, pickup date and instructions, course interests, account-opening details, referral source, and the message supplied by the visitor.
Security systems may also process limited technical information, including IP address, browser and device details, request time, requested page, and events needed to detect abuse, protect the site, and diagnose delivery failures.
How we use information
- Respond to the request and communicate with the practice or organization.
- Arrange an eligible local pickup or other requested logistics.
- Review a practice-account, education, partnership, or general inquiry.
- Protect the website, prevent repeated or fraudulent submissions, and investigate service problems.
- Maintain internal records needed to complete the requested business process and meet applicable obligations.
Phone numbers and SMS
Phone numbers entered in YM's public forms are used for operational contact about the request. Submitting a contact, pickup, course, or practice-account form does not enroll the visitor in marketing text messages.
Mobile information will not be shared with third parties or affiliates for marketing or promotional purposes. Text-message opt-in data and consent will not be sold or shared with third parties for their own marketing. Service providers may process that information only to deliver an approved messaging service on YM's behalf.
Any text-message program requires a separate, optional, affirmative opt-in and the disclosures described in the SMS Messaging Terms. A general website form is not text-message consent.
Analytics and cookies
Analytics and advertising tracking are disabled for the planned website launch. The site may still use storage that is strictly necessary for security, form integrity, accessibility, or a feature the visitor directly requests.
YM will update this notice before enabling nonessential analytics, advertising tags, or related cookie choices.
Retention and security
YM limits access to submitted information and uses technical and organizational safeguards intended to protect it. No website or transmission method can be guaranteed completely secure.
YM retains submitted information only as long as reasonably needed for the requested business process, security, recordkeeping, and applicable legal obligations. Related backups and security records may remain for a limited period after the active record is removed.
Your choices
You may choose not to submit a public form and instead call YM Dental Lab. You may also ask YM about information provided through the public website, request a correction, or ask what deletion options are available. Some records may need to be retained for security, legal, or legitimate operational reasons.
